Policies and Procedures
We strive to provide compassionate, individualized care in a practice that is organized and responsive. The policies and procedures that follow help us communicate clearly, provide care efficiently, and ensure a smooth experience for everyone.
Appointments/Office Hours: Dr. Camp sees patients by appointment only, Monday through Thursday. Appointments may be scheduled or rescheduled through the patient portal or by leaving a message at the main office number, 469-210-1235.
Communication with the Practice: The preferred method of communication regarding your care is through the patient portal. However, you may also contact the office by leaving a voicemail at 469-210-1235. Both patient portal messages and voicemail messages are monitored periodically during normal business hours, Monday through Thursday, 9:00 a.m. to 5:00 p.m. The office is closed on Fridays. We aim to respond to non-urgent messages within one business day. Messages received after 5:00 p.m. on Thursday or at any time on Friday will typically be addressed on Monday.
The patient portal and voicemail should not be used for urgent or emergency concerns. If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency department. If you have an urgent concern that cannot safely wait until the next business day but is not an emergency, you may call the office and follow the instructions to reach the after-hours answering service. Based on the nature of your concern, the answering service may contact Dr. Camp. After-hours calls that require a response before the next business day may incur a $50 after-hours fee. Routine questions, medication refill requests, scheduling issues, and other non-urgent matters should be addressed during normal business hours.
Follow-Up Visits: Dr. Camp will recommend follow-up appointments based on your clinical needs and treatment goals. The frequency of visits varies according to individual needs and patient preference. Most patients are seen every 1–3 months, although less frequent follow-up may be appropriate for some.
Patients who have not been seen for more than one year are no longer considered active patients and will need to schedule a new patient evaluation to re-establish care.
Missed or Late-Canceled Appointments: The practice requires at least 24 business hours' notice to cancel or reschedule an appointment. Business hours are Monday through Thursday, 9:00 a.m. to 5:00 p.m. Appointments that are missed, canceled or rescheduled without the required notice will be charged the full appointment fee.
Medication Refills: Dr. Camp will make every effort to prescribe enough medication to last until your next scheduled appointment. If you cancel or reschedule your appointment, it is your responsibility to notify the office if you will need additional medication before your next visit. Medication refills are provided only for patients who are actively engaged in treatment and have a scheduled follow-up appointment. Please allow up to 48 business hours for refill requests.
Telemedicine Appointments: Telemedicine appointments are available when clinically appropriate and are provided through a secure, HIPAA-compliant video platform. The decision to conduct a visit by telemedicine is based on clinical judgment and may vary depending on your condition and treatment needs.
Patients are responsible for ensuring they have a reliable internet connection, a device with audio and video capability, and a private location where confidentiality can be maintained. If technical difficulties prevent completion of a video visit, the appointment may be converted to a telephone visit, rescheduled, or otherwise handled at Dr. Camp's discretion.
Patients must be physically located in the state of Texas at the time of their telemedicine appointment unless otherwise permitted by applicable law and licensing requirements. By participating in a telemedicine visit, you confirm that you are in Texas and will provide your current location and a telephone number at the beginning of each appointment in case of an emergency or technical interruption.
Insurance Policy: Camp Psychiatry, PLLC is an out-of-network practice and does not accept insurance. Payment is due at the time of service and will be charged to the credit card on file unless another payment method has been arranged in advance.
Upon request, the practice can provide a superbill for patients who wish to seek out-of-network reimbursement from their insurance plan. Reimbursement is determined solely by your insurance carrier and is not guaranteed. Please note that Medicare beneficiaries may not submit claims to Medicare for services provided under a private contract.

CAMP PSYCHIATRY, PLLC’S RESPONSIBILITIES TO YOU AND YOUR INFORMATION:
Camp Psychiatry, PLLC is required to:

REQUESTING COPIES OF YOUR MEDICAL RECORD:
You may inspect and/or copy health information that is included in your medical record. Texas law requires that requests for copies be made in writing. We can refuse to provide some of the information you request to review for the following reasons:
• The information includes psychotherapy process notes
• The information has been compiled in anticipation of litigation
• The information, in the judgment of your treating provider, could cause mental harm to you. In this case, a summary of your care will be provided instead.
All documentation completed by your provider and included in your medical record can be provided to you electronically at no cost to you. If you request printed copies of your medical record from Camp Psychiatry, PLLC, you will be charged $0.10 per page, up to a maximum of $50, for your records. Under Texas law, we are required to provide you with a copy of your medical record within 15 days of your written request.
REQUESTING CORRECTIONS OR AMENDMENTS TO YOUR MEDICAL RECORD:
You may request an amendment to your medical record in writing. We will respond within 60 days of your request. We may refuse to allow an amendment for the following reasons:
• The information was not created by Camp Psychiatry, PLLC
• The information is not part of your medical record
• The information is not available for inspection because of an appropriate denial
• The information is accurate and complete, as determined by Camp Psychiatry, PLLC
Even if we refuse to allow an amendment, you are permitted to include a patient statement about the information at issue in your medical record. If we refuse to allow an amendment, we will inform you in writing. If we approve the amendment, we will inform you in writing and appropriately amend your health information.
REQUESTING SPECIFIC METHODS OF CONFIDENTIAL COMMUNICATION:
You have the right to request that Camp Psychiatry, PLLC communicate with you in specific ways – for example, by calling a specific phone number or by refusing specific forms of communication. We will agree to all reasonable requests that you make.
When communicating with us, we recommend utilizing specific forms of communication, including calling Camp Psychiatry, PLLC office and using the patient portal, as these methods of communication are compliant with industry-standard privacy practices. While you have the right to refuse to allow us to communicate with you in these ways, doing so may reduce our ability to maintain the confidentiality of your health information.

REQUESTING RESTRICTIONS ON THE USE OF YOUR INFORMATION:
By default, your information will be shared in the following ways, which are necessary for us to provide care to you:
• Your information will be shared with you unless specifically withheld under a listed exception above
• Your information will be shared with other members of Camp Psychiatry, PLLC staff. All staff at Camp Psychiatry, PLLC are “Covered Entities” under federal law and are required to maintain the confidentiality of your health information as described in this notice. This allows Camp Psychiatry, PLLC staff to contact you about your care and receive information from you when you contact Camp Psychiatry, PLLC. Camp Psychiatry, PLLC staff will access your health information only if necessary to provide care to you.
While you have the right to refuse to allow Camp Psychiatry, PLLC to share information in these ways, your refusal may limit our ability to provide care to you.
EXCEPTIONS TO CONFIDENTIALITY:
There is some information that Camp Psychiatry, PLLC is legally mandated to share with other parties, with or without your consent. In these cases, Camp Psychiatry, PLLC will share only the minimum information necessary to comply with state and federal law. These disclosures include:
• Information related to active, ongoing abuse of a minor. By law, child abuse must be reported to state officials.
• Information related to active, ongoing abuse of an adult 65 years or older or an adult with disability. By law, elder and dependent-adult abuse must be reported to state officials.
• Information related to a new diagnosis of certain sexually transmitted infections (STIs) made by a Camp Psychiatry, PLLC provider. By law, these diagnoses must be shared with state and county public health officials for public health purposes.
• Information requested under legal compulsion due to a lawful judicial order. We will attempt to resist any legally compelled disclosure of your health information, and any disclosure will be the absolute minimum required under a judicial order.
• Information required to defend the practice or a provider against a lawsuit or other legal action
Additionally, Camp Psychiatry, PLLC is permitted, but not required, to share information about you in situations in which, in the judgment of your provider, sharing information about you may lessen imminent dangerousness to you or others. These disclosures can occur with or without your consent, if necessary. In these cases, only the minimal amount of information about you will be shared.
AUTHORIZING RELEASE OF YOUR INFORMATION:
Other than the exceptions described above, your health information can be shared with other parties only with your written consent. If you would like your Camp Psychiatry, PLLC provider to speak with other parties about your care – for example, your spouse or other family members – you must complete and sign a release of information form with Camp Psychiatry, PLLC . This form will describe with whom your information may be shared with and what information will be shared. You may revoke this authorization at any time.
FILING A COMPLAINT ABOUT OUR PRIVACY PRACTICES:
If you have any questions or concerns about how Camp Psychiatry, PLLC is handling your information, we encourage you to contact our office so that we have the opportunity to address any concerns that you might have. However, you may also send a written complaint to the U.S. Department of Health and Human Services’ Office of Civil Rights. We will not take any action against you or change the care you receive if you choose to file a complaint.
